The Warm Health Care Management Program Suite offers a library of comprehensive, condition specific programs that are unique within the industry. The suite is designed to address important challenges for health plans including chronic disease management, maternal health management and hospital discharge.

Our Approach to Care Management

Warm Health’s Care Management approach extends and focuses care manager outreach through automated, interactive phone calls, texts or emails to members. Warm Health improves health outcomes through increased member contact by delivering micro-education (health topics that are broken up into multiple, short messages) and by monitoring members to connect at-risk members to care managers.

Members across the entire risk pyramid are contacted at risk-adjusted frequencies with education and questions that determine the care management needs of each member. Warm Health technology identifies members with potential problems and alerts care managers. Care managers can then proactively contact the member to assess the situation and avoid an escalation of health problems and increased medical costs.

Care Managers have access to the Warm Health Care Dashboard, which provides real-time information on member outreach and a method to track the results of care management follow-up to Warm Health alerts. Members can also be enrolled in programs via the dashboard or through data files exchanged with the health plan. Additionally, care managers can adjust the configuration of currently enrolled member using the Warm Health Dashboard.
